6017db9dc8 Add missing urlencoding to base/webroot. 
This fixes URL generation when PHP_SELF or the request path contains
special characters.

65b1a94e63 Simplify how fullBase is calculated. 
Using FULL_BASE_URL fixes URL generation when URL rewriting is
disabled.

1f35d82c81 URL encode image/script/css/video paths. 
Properly urlencode urls used in HTML attributes. This solves issues with
invalid HTML being generated when paths contain special characters.

Fixes #3395

2c0e030831 Move call to tagIsInvalid to FormHelper 
tagIsInvalid() doesn't exist in Helper, and shouldn't be
called from that class.
